Certified Home Health Aide (CHHA)

*Certified Home Health Aide- Full-time* *Requirements:* * Completed a Certified Nursing Assistant (CNA) training program and have an active license * Active CPR Certification * 1 year of relevant experience as a CNA * Compassion, integrity, and a commitment to providing excellent care * The ability to handle situations with confidentiality and maturity * The ability to read, write, and speak English * Commitment and dependability * Rotating weekend and holiday on-call responsibilities *Primary Job Responsibilities:* * Care for patients in their homes in conjunction with the rest of the clinical team.
* Follow a personalized care plan for each patient that is assigned by the case manager who is available to support you in your responsibilities.
* Assist with activities of daily living (ADLs), light meal preparation, companionship, respite when applicable.
* Promote a safe environment and assist with ambulation/transfers.
* Maintain communication with the interdisciplinary team and the patient’s caregivers.
*All employees must be vaccinated with booster for COVID-19.
* Job Type: Full-time Pay: From $18.
